The following forms/links are mandatory for volunteering and field trips:

Safe Environment Certification on Virtus (3yrs)
Livescan/Fingerprinting (lifetime) - form will be provided by the school once you register and train on Virtus
TB Clearance (4yrs)
Volunteer Agreement Form (every year)

**Please Note: Registering on Virtus must be done first

School Reimbursement Request:

To be reimbursed for any expenses incurred on behalf of St. Mary School, please fill out the reimbursement form and submit it to the office with receipts attached. Reimbursements must be approved by the principal before being processed by the school bookkeeper.
